First, you might want to print out these instructions.

You can easily get a form that fills a standard 8-1/2 x 11 inch sheet of paper if you are familiar with saving just the image or picture of the form (the .gif file) to your hard drive. Then open the file with your graphics application, and print it. You should get a form that fills a standard 8-1/2 x 11 inch sheet of paper.
